Right after that, Real Madrid confirmed that James has a bone injury that could be a fracture.

The Colombian player will get X-rays on his foot to see how serious the injury is, but he will almost surely miss the Derby against Atlético this Saturday. Ramos' injury is not as serious, and depending on how his injury develops, he could try to start at the Calderón.
Ancelotti will now need to tweak his starting XI, because if the fracture is confirmed, James could be out for at least two months, meaning that Luka Modric's recovery needs to go better than expected now in order for Real Madrid to replace James properly.
